AFAIK they are supposed to be elite boss variants, but I don't think it is actually possible to beat 20 elite mobs required to spawn them with what little sanity they give you. It’s annoying but it’s not guaranteed death at full insanity. You can grind through it. Just dodge the ghosts, i managed to spawn naricain on bent tree.
